marriage2Each party seeking a marriage license should be prepared to supply the following information: full name, residential and mailing address, phone number, age, date of birth, race, social security number, birthplace, relationship of parties, if any, designated surname (last name), number of prior marriages and how they were dissolved, if any, father's name and birthplace, mother's maiden name and birthplace, and the date and place of the contemplated marriage.

Parties have a legal obligation to provide current, correct, and full legal names on the marriage license application.
