Each party seeking a marriage license should be prepared to supply the following information: full name, residential and mailing address, phone number, age, date of birth, race, social security number, birthplace, relationship of parties, if any, designated surname (last name), number of prior marriages and how they were dissolved, if any, father's name and birthplace, mother's maiden name and birthplace, and the date and place of the contemplated marriage.
Parties have a legal obligation to provide current, correct, and full legal names on the marriage license application. If the information provided on the marriage license application is not correct, the applicants may have problems with other government agencies, such as Immigration, Social Security, or Driver’s Services, and may incur additional costs and time delays when seeking to have the application for marriage license corrected.
